- A typed non-immigrant visa application Form DS-156.
- Form I-797: The latest INS approval notice. This has information on the applicant's current employer and indicates the approval dates or renewal / extension dates.
- Employer's Letter of Support addressed to the Visa Office of the Department of State containing information related to applicant’s position and type of business.
- A self-addressed or stamped envelope for the return of passports.
- Filing fee in the form of check/ money order, payable to the Department of State.
